Liver SBRT delivers ablative doses — 45–60 Gy in 3–5 fractions — to tumours that sit millimetres from the bowel, portal vessels and biliary system. The planning geometry must be correct not just at simulation, but during every second of beam-on time across all fractions.

Soft-tissue CBCT matching in the liver introduces a mean positioning error of 5.3 mm. Fiducial-based registration reduces this to 1.5 mm — a 3.5-fold reduction published in a prospective SBRT series.

Real-time Tracking

SyncTraX + Gold Anchor — gated liver SBRT

SyncTraX is a real-time tumour-tracking radiotherapy system that uses X-ray fluoroscopy to track implanted fiducial markers during beam delivery. For liver SBRT, Gold Anchor markers provide the reliable, migration-free reference point that this system requires.

A 2023 publication in the Journal of Radiation Research validated real-time gated liver SBRT with SyncTraX in patients where isocenter shifts were required — a technically demanding scenario that requires complete marker stability throughout the tracking session.
