Q.Why does sterile radiopharmaceutical compounding need a vertical laminar flow hood rather than a horizontal one?

Sterile compounding of radiopharmaceuticals (Tc-99m kit reconstitution, Ga-68 peptide labelling, Lu-177 conjugation) needs Grade A airflow directly onto the compounding surface. A horizontal LAF blows air from the back wall across the work zone and out into the operator's face — carrying any aerosolised activity from the vial straight at the operator. A vertical LAF pushes air down from a HEPA-filtered ceiling to the work surface and out through side vents, keeping the operator out of the exhaust path. For radioactive work the vertical geometry is the safer default.

Q.Why lead-lined instead of a standard clean-room vertical LAF?

A standard vertical LAF handles sterile chemistry but has no radiation shielding. Radiopharmaceutical compounding puts activity right on the work surface where the operator's hands are — Tc-99m at 140 keV, Ga-68 at 511 keV, Lu-177 β + γ. Lead lining in the back wall and side panels attenuates the source dose to the operator's body during the compounding cycle. Combined vertical-flow sterility + lead-lined radiation shielding is what a hot-lab compounding station actually needs.

EU GMP Annex 1 sets the Grade A airflow specification, HEPA filtration class, particle-count monitoring and cleaning-validation schedule for sterile compounding.
