Thermoplastic Immobilization Mask FamilyS-Frame H&N · Full-Face Cranial + SRS · Full-Body Shell
Three-SKU thermoplastic mask family covering the whole external-beam radiotherapy anatomy — S-Frame head-and-neck (RT-001), full-face cranial + intracranial SRS (RT-002), full-body thoracic shell (RT-003). Low-temperature 65 °C activation bath softens the perforated polymer sheet at simulation; the sheet moulds to the patient, hardens as it cools, and locks the patient shape into the same shell every fraction. Indexed to standard couch-bar-pitch baseplates; MR-compatible fastener set available for MR-linac + MR-sim workflows. Peer-reviewed setup accuracy across every workflow the family covers — sub-2 mm 6D translational error under daily MVCT (You 2024), sub-millimetre systematic error on MR-Linac (Cuccia 2021), and quantified SRS intrafractional drift plateau at ~ 10 min under mask fixation (Mangesius 2019).

Applications
Six worked treatment workflows the mask family covers
Routine head-and-neck IMRT / VMAT
RT-001 S-Frame mask on a standard H&N baseplate + shoulder-retraction strap. Daily MVCT / CBCT verification confirms sub-2 mm mean 6D translational setup error (You 2024 · 30 patients / 920 images). The workhorse configuration for the majority of the departmental H&N caseload.
Intracranial SRS / SRT
RT-002 full-face SRS mask + O-frame + SRS bite-block. Intrafractional 3D drift 0.21 mm (0-2 min) → 0.53 mm plateau at 10 min under mask fixation (Mangesius 2019 · 96 fractions). Sets the SRS beam-on ceiling per fraction under AAPM TG-42 < 2 mm targeting.
MR-Linac + MR-guided radiotherapy
RT-001 or RT-002 + non-ferromagnetic fastener set + MR-compatible baseplate. Multi-centre European review confirms < 1 mm systematic translational error with the MR-compatible thermoplastic mask + coil-compatible workflow (Cuccia 2021).
Paediatric brain-tumour SRT
RT-002 paediatric-size full-face mask + O-frame + paediatric bite-block. Same physics as the adult SRS configuration but sized for paediatric anatomy; the low-temperature activation avoids the moulding-time burn risk that would otherwise limit paediatric use.
Trunk / thorax IGRT + IMRT
RT-003 body shell + indexed thorax baseplate + arm supports. The body-shell restrains trunk motion across the fraction series; pairs with the thermoplastic head mask (RT-001 or RT-002) for the H&N-and-nodal treatment plans that need dual immobilisation.
Customised 3-point re-irradiation immobilisation
RT-001 or RT-002 + custom cushion + SRS bite-block. MD Anderson published a 21-patient / 105-session H&N SRT re-irradiation cohort where this configuration corrected initial 2.7 ± 1.4 mm 3D setup vectors to 0.0 ± 0.3 mm per-axis residuals — 2.0 mm PTV margin achievable (Wang 2016 · JACMP).
Why Thermoplastic Immobilization Mask Family?
The perforated polymer softens in a 65 °C water bath — well below the patient burn threshold. The RTT moulds the softened sheet directly to the patient at simulation; the sheet cools and hardens into a patient-specific shell within minutes. No pre-scanning, no third-party cast fabrication, no simulator turn-around delay.
A 30-patient / 40-mask / 920-image daily-MVCT H&N cohort showed mean 6D translational setup errors of 0.32 mm (X), −1.09 mm (Y) and 2.24 mm (Z) — well inside the CTV-to-PTV 3-5 mm margin used in standard H&N IMRT. Rotational errors below 0.4° on every axis. That's what a modern thermoplastic-mask workflow delivers on the daily image-guided cadence.
A multi-centre European review anchors the MR-compatible thermoplastic mask + non-ferromagnetic fastener + coil-compatible-baseplate workflow at < 1 mm systematic translational setup error on MR-Linac + MR-sim. The same shell transfers from CT-sim to MR-sim to MR-Linac without per-room recalibration.
Variant matrix
Pick the SKU, the frame and the accessory kit
Three SKUs cover the whole external-beam anatomy — head-and-neck IMRT, cranial SRS and trunk IGRT. Frame compatibility spans S-frame, SRS-frame and indexed thorax baseplate. Accessory kit maps the mask to the treatment workflow (H&N routine, SRS re-irradiation, MR-Linac).
SKU variants
| SKU | Variant | Fit | Frame | Use / evidence |
|---|---|---|---|---|
| RT-001 | S-Frame head-and-neck mask | Head + shoulder | S-frame base plate | Routine H&N IMRT / VMAT; sub-2.24 mm mean 6D setup error under daily MVCT (You 2024) |
| RT-002 | Full-face cranial + SRS mask | Cranium + face | S-frame or SRS-frame compatible | Cranial IMRT + intracranial SRS / SRT; intrafractional 3D drift 0.21 mm at 0-2 min → 0.53 mm plateau at 10 min (Mangesius 2019) |
| RT-003 | Full-body thermoplastic shell | Trunk / thorax | Indexed thorax baseplate | Trunk / thorax IGRT + IMRT; pairs with indexed thorax baseplate for reproducible pelvic + abdominal setup |
Frame + baseplate compatibility
| S-Frame | RT-001 head-and-neck workhorse; standard couch-bar-pitch indexing; compatible with the majority of modern linac head-support baseplates |
| O-Frame / SRS-Frame | RT-002 stereotactic use; extends the S-frame concept with tighter head-shell fit for < 2 mm targeting SRS work under AAPM TG-42 |
| Indexed thorax baseplate | RT-003 body-shell interface; couch-bar-pitch indexing matches the thorax baseplate from the Patient Positioning Solutions family |
Accessories Matrix
| Mask-water bath (65 °C thermostat) | Low-temperature activation bath softens the thermoplastic sheet for moulding to the patient at simulation; thermostat holds the working range without operator supervision |
| Mould-room work surface + tools | Scissors, thermal gloves + reshaping tool for the moulding step; the mould-room fit is done at simulation and the mask ships to the treatment room with the patient plan |
| Shoulder-retraction strap | H&N-specific accessory paired with RT-001 — reproduces the shoulder-down position across fractions; reduces the neck-flexion setup uncertainty that would otherwise absorb margin |
| SRS bite-block (H&N + cranial) | Additional intra-oral fixation for stereotactic use (RT-002); the customised 3-point cushion + mask + bite-block combination has been shown to bring 3D residual setup errors to ~ 0.0 ± 0.3 mm per axis (Wang 2016 · MD Anderson) |
| MRI-compatible fastener set | Non-ferromagnetic hook / lock hardware for MR-linac + MR-sim workflows; the mask itself is polymer and MR-safe by default |
| Patient-comfort cushion | Between the mask and the skin — reduces pressure points across a 20-30 fraction series; does not compromise the shell fit or the setup reproducibility |
| Storage rack (per-patient labelled) | Between-fraction storage in the treatment-room ante-space; each patient's mask is labelled with treatment-plan ID + fraction schedule |
| Per-batch material + moulding certificate | ISO 13485-aligned material record — passive-accessory dossier that ships alongside the medical-device import paperwork under CDSCO Medical Devices Rules 2017 |
Per-batch material + moulding certificate ships with every consignment. MR-compatibility fastener set is specified at ordering. Family configuration + accessory kit selected against the receiving centre's treatment mix during site IQ / OQ / PQ.
